This form is to request that the Department of Dance provide a copy of the video of the performance described below. I attest that I will use the copy of the video provided to me only as specified in both this Department of Dance Copyright Policy and the permission form from the choreographer/director for this performance.
Timeline of Requests
During the academic year, requests can be expected to be filled within two (2) weeks. During the summer or academic breaks, the request may not be filled until the academic year is back in session. Rush requests are available but are not guaranteed to be accommodated. Sharing, use, and methods of dissemination are dependent on the choreographer/directors' permissions and all credits must be given. If you are requesting more than one video, a separate form should be filled out for each unique request.
Archival Requests
A large archival project was completed around 2020 which includes photos and video from the Department of Dance pre-1960 to mid 2010’s most of which can be found on the OSU Dance Digital Archive Webpage. If you are curious if an event that was held before 2011 is digitized, that website is an excellent and up to date resource. For items post 2011, requests can be made via this form and a dance media team member will check and see if it is available. Please note that requests for digitization of archival items not on the OSU Dance Digital Archive Website are not guaranteed at this time or may incur a very long turnaround time due to staff capacity. Copyright Policy of the Department of Dance at The Ohio State University
All choreographers/directors (including students, faculty and guest artists) must determine if copyright permission is needed to use any music and other media in their work presented in performance by The Ohio State University Department of Dance and must obtain copyright permission where required by law. Upon request, video of performances taken by the Department may be released to students, faculty, or guest artists who participated in the performance. All performers and/or designers must have signed permission from the choreographer/director to obtain video and follow the permissions outlined on that form. Requestors are expressly forbidden from publicly displaying videos taken by the Department, including posting these videos on Internet sites, if they do not have the needed copyright permissions. Please note that copyright permissions for live performance may not be the same for dissemination and it is the requestor's responsibility to research and obtain said copyright if needed.
In addition to copyright, it is best practice to credit all involved parties (performers, technicians, costume designers, etc.) when appropriate. This is at the requestor's discretion depending on the use of the video requested, but highly encouraged.
